Right Care, Right Place: Nursing Leadership in Care Transformation Across Settings

As health systems rethink where and how care is delivered, nurse leaders are advancing models that extend care beyond traditional acute care settings. This webinar will examine two approaches: integrated Hospital at Home and Home Health services that help patients receive care at home, and a 24/7 oncology care model that supports patients across urgent, procedural, cellular therapy and observation-level needs. Presenters will share how these models support continuity, access, patient satisfaction and operational flow while reducing infection risk, avoidable readmissions, emergency department boarding and unnecessary inpatient utilization. The discussion will focus on leadership, coordination and implementation strategies that make these care models safe, sustainable and patient-centered.

About the Presenters

Jillian Tuzio, DNP, RN, AGCNS-BC, OCN, is the Clinical Director of Oncology Nursing at the Hospital of the University of Pennsylvania, where she provides leadership for over 480 nurses and support staff across the oncology service line. Her leadership spans the full continuum of cancer care, from advanced cellular therapies and allogeneic transplantation to supportive and palliative care services. Jillian earned her Doctor of Nursing Practice degree from Villanova University, where her scholarship focused on evaluating the impact of nurse-led palliative care screening for oncology patients.

Marie Iannelli Carrera, MSN, CRNP, is the Associate Manager of the Oncology Evaluation Center, where she has spent nine years caring for patients with urgent oncology needs. For the past two years, she has supported the center’s advanced practice provider team, providing leadership for daily operations and clinical practice. Before becoming an advanced practice provider, she worked as a registered nurse in the emergency department.

Katie has served Advocate Health in operations, quality and clinical roles since 1989, with positions including executive director for home health services and vice president of clinical services. She is responsible for the overall strategic direction of clinical programs, practice and quality to drive success in a value-based environment. Her responsibilities also include acting as chief patient care officer for regulatory, licensing and accreditation, evaluating and improving the quality and appropriateness of care, promoting patient safety through the principles of a high-reliability organization and integrating virtual health programs in work streams. 

Julia Jeter has been with Advocate Health for 14 years, serving in roles as a nurse, implementation specialist and clinical leader. Julia has played key roles in Primary Care redesign, Atrium Health’s Primary Care Connect program and the enterprise expansion of Hospital at Home services. She currently serves as Vice President in the Continuing Health division, leading Enterprise Hospital at Home and the NC/GA Division’s Post Acute Network SNF program advancing system care-in-the-home strategy, problem solving and clinical innovation across inpatient and outpatient settings.
