Community Health Improvement Week is June 1-5. During this week, we’ll be sharing stories about how hospitals, public health and community organizations, and individuals are coming together during this challenging time to protect the health of their communities. Stories can be submitted in a variety of formats, including narratives, video and images.

By submitting this information, I represent that I have obtained a valid HIPAA Authorization covering all future disclosures AHA may make of this information if the disclosure includes any protected health information of the patient.
