AHA brief highlights how to improve care coordination
The American Hospital Association released a brief explaining how hospitals and health systems can incorporate care navigation and care coordination into their patient care processes. The brief covers key aspects, use cases, implementation considerations and information about additional resources. It stresses the importance of getting buy-in from the board and C-suite leadership, as well as gathering insights from nurses, physicians and administration staff on what they hear from patients and care gaps they observe. The brief suggests using patient surveys or focus groups to design and improve programs. It also encourages leaders to use community organizations and available population-level data to design care navigation and coordination and inform a more comprehensive understanding of community needs.
