AHA releases care delivery transformation resources

The American Hospital Association launched a Care Delivery Transformation Framework to provide hospitals with tools and resources to develop and implement innovative care models. The framework provides a visual representation of how the components of the health care ecosystem facilitate the development and implementation of innovative care delivery models. Care delivery requires collaboration and goal alignment across all components of the health care system. By leveraging existing tools, hospitals can integrate new technologies, team members and strategies to design and implement care delivery models that improve the health care experience for patients and providers.
