AHA webinar addresses patient social, economic needs

The American Hospital Association will host a webinar on providing whole-person care by addressing patients’ social and economic well-being on June 26 from 2:00 to 3:00 p.m. ET. MaineHealth and Children’s Hospital Colorado representatives will discuss their tailored approaches to build infrastructure to follow-up meaningfully with patients who have responded affirmatively to questionnaires identifying social and economic needs. MainHealth’s interprofessional team collaborates with more than 200 community-based organizations to ensure timely and effective responses to identified patient and community needs. Children’s Hospital Colorado provides an onsite hub of partners to facilitate handoffs and streamline access to support. Presenters will share strategies for implementing and sustaining follow-up efforts in inpatient and outpatient settings. Participants should register in advance.
