Northwell Health describes how it implemented age-friendly care
In an American Hospital Association podcast, Northwell Health leaders discuss how the system transformed age-friendly care into an organization-wide strategy. The New Hyde Park, N.Y. system’s chief financial officer and vice president for aging and supportive care collaborated to identify quantitative metrics such as reduction in length of stay, falls and readmissions to demonstrate the return on investment for providing age-friendly care. They implemented age-friendly care at one site to identify improvements in patient care and overall savings before scaling the program. They said the strong relationship between finance and clinical care enabled Northwell to implement the program in a cost-effective and efficient manner. The board is heavily involved in reviewing metrics, which include reduced lengths of stay and readmissions and increased patient satisfaction.
