Sutter Care at Home pilots Age-Friendly Health System framework

Sutter Care at Home participated in the Age-Friendly Health Systems prototyping initiative to use the 4Ms Framework in homecare. Sacramento, Calif.-based Sutter Health tested the 4Ms Framework: What Matters, Medication, Mentation and Mobility at its homecare division in Yuba City, Calif. Leaders used interprofessional collaborative education to target specific team needs and make minor changes to care processes, leading to an increased assessment of the 4Ms from 10% to 60% of older adults during the prototype period. Several team members also reported a greater connection to purpose. Sutter Health expanded the program to all of its home care divisions. The Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation, the Institute for Health Improvement and the American Hospital Association.
