Use of latent safety threats explored

Researchers at Advocate Health Care made systemwide improvements to patient safety by classifying latent safety threats ─ characteristics of design, processes or physical environment in health care compromising patient safety ─ identified during simulation-based training. The Charlotte, N.C.-based health system researchers collected 1,318 LSTs systematically in 232 simulations over one year and developed a taxonomy to classify them. Based on their findings, the researchers issued site-specific and organizationwide standardized dashboards and summaries to make systemwide improvements.
