Advocate Health uses AI-outreach tool for patients missing follow-up appointments
Advocate Health deployed a conversational artificial intelligence tool to reach 15,000 patients hospitalized for elevated blood pressure who missed their follow-up appointments. Nurse informatics leaders and nurses at the Charlotte, N.C.-based health system built an AI tool that explained the importance of monitoring hypertension, discussed how to check blood pressure at home and report readings to their care teams. It gave patients information about places to check their blood pressure in their communities and report results. People who couldn’t report a blood pressure screening connected with nurses who helped them. Advocate says patient surveys were positive. The system may expand the tool to reach patients with heart failure, diabetes and chronic obstructive pulmonary disease. (HealthLeaders Media article, 6/1/26)
