AHA case studies show how hospitals narrow health disparities

The American Hospital Association released eight case studies showing how hospitals and community organizations have improved their collaborative efforts using the Hospital Community Collaborative tools and resources, ultimately reducing disparities in health outcomes in their communities. In addition, three experts from Phoenix area-based HonorHealth discuss in an AHA podcast how their health system is addressing community needs beyond traditional care, such as food insecurity and through offering an Adult Day Health Care Center. Another AHA podcast features a representative from the Cleveland Clinic discussing how the medical center addresses food insecurity by treating food as a vital part of health care. Combining data, community trust and local partnerships, the Cleveland Clinic promotes long-term well-being.
