Webinar shows age-friendly care aided by community partnerships

An Age-Friendly Health Systems webinar on June 15 at 12:00 p.m. CT will describe a framework to address older adults’ unique needs, as well as how to integrate age-friendly care in the community. Monica Long, MSN-Ed, RN, a nurse educator from the SHARE Network, will discuss how her team is implementing age-friendly care successfully through community partnerships. Participants also will learn how to participate in the American Hospital Association’s Age-Friendly Health Systems Action Community convening this fall. Participants must register in advance for the webinar. Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States.
