House subcommittee discusses care coordination to prevent, treat chronic disease
The House Ways and Means Subcommittee on Health held a hearing to discuss improvements to care coordination and delivery to prevent and treat chronic disease. Health care and pharmaceutical experts testified before the committee, including Michael Hoben, MD, chief medical officer of population health services at Winston Salem, N.C.-based Novant Health. In written testimony, Hoben described how coordinated, patient-centric care prioritizes patient safety, high-quality care and outcomes, wellness and disease prevention, affordability and accessibility. His comments emphasized that when policies and practice are designed to enable multifaceted collaboration, affordability and efficiency, patient well-being is prioritized and health care workers can devote their attention to caring for patients. Among the initiatives Hoben discussed are Novant’s virtual care offerings, mobile clinics and hospital-at-home program. (AHA News article, 11/19/25)
